rendered. Patient is responsible for charges not

the insurance will cover services. |

Can I get a copy of my lab report?

No, due to regulations lab results
are sent directly to your ordering physician. |

Why is my account going to collection?

Your account was not paid within
90 days. Please contact the collection agency
to settle your claim. |

Do you do courtesy testing?
